Kathy Denise Johnston, 54, Phil Campbell, passed away Wednesday, April 18, 2012, at her residence.

The family will have a private graveside service at Limerock Church of Christ Cemetery.

She is survived by her father, Willard Mayfield; son, Billy Wayne Clark; two granddaughters, sister, Regina Mayfield; brother, Willard Mayfield, Jr., and wife, Jennye; and several nieces and nephews.

She was preceded in death by her mother, Jean Tittle Mayfield; and son, Stevie Ray Mayfield.

The family would like to thank Joe Hester for the professional care he showed towards the family.
