Jerry D. McCulloch, 69, Russellville, passed away Thursday, May 10, 2012, at Haleyville Healthcare.

Visitation will be Saturday, May 12, 2012 from 6-8 pm at Akins Funeral Home, Russellville. The funeral service will be at 2 p.m. Sunday, May 13, 2012, at Akins Funeral Home, Russellville, with Bro. James McCullar officiating. Interment will be in Spruce Pine Cemetery.

Pallbearers will be Kenneth Cummings, Larry Rickard, Eric McDaniel, Ricky McDaniel, Josh Kelley, and Tom Willis.

He was preceded in death by his parents, Jessie and Bannie McCulloch; brothers, Dennis McCulloch and Jim McCulloch; brother-in-law, Jessie Borden.

He is survived by children, Shelia McGuire and husband, Jimmy, and Timothy Dale McCulloch; siblings, Juanita Borden, Mamie Lee Scott; grandchildren, Kelsey Motes Jr. and wife, Mary, Tisha  Kelley and husband, Josh, Trenton McCulloch, Tre McCulloch; great grandson, Isaiah Lee Kelley; mother of his children, Patricia Armstrong; special companion, Charlene Wilson.

He was a member of the Baptist Church. He was an accomplished legendary musician, skilled in playing 11 different instruments.

Special thanks to Southern Care Hospice, especially nurses Tina Mauldin, Amy Faucett and Tammy Garret.
